    Since 1951, Episcopal Retirement Services (ERS) has worked to improve the lives of older adults through innovative, quality senior living communities and through community services to older adults.

     

    The primary purpose of the Care Coordinator, working directly under the supervision of the Geriatric Care Manager,  is to help coordinate services within the parameter of the Care Plan for  the client, family and other care and service providers. The Care Coordinator may assist the Geriatric Care Manager with initial telephone contact for new clients, various office responsibilities and other duties as requested.

    Responsibilities

    • Demonstrates sensitivity to client and/or family needs, customs and feelings and to the greatest extent possible fosters self-determination on the part of the older person to facilitate living in accordance with his/her personal values and goals. 
    • Provides companion services, errands, activities, light housekeeping, laundry and cooking as scheduled and according to Care Plan.
    • May communicate with providers indicated in care plan in conjunction with the client.  Communicates effectively with family members, physicians and client’s other providers when requested.
    • Provides a safe environment by recognizing emergency situations and implementing proper procedures.
    • Provides ongoing performance feedback and input to performance appraisals for preferred providers when asked.
    • Adheres to all of ERS’s policies and procedures including our safety and Corporate Compliance programs.
    • Maintains confidentiality of a resident’s pertinent health and financial information in accordance with HIPPA guidelines.

    Qualifications

    • Minimum one year of experience working with elder patients/families in a care giving role.
    • Excellent listening and communication skills.
    • Excellent documentation skills.
    • Must be able to work independently with little supervision and able to handle multiple tasks while meeting deadlines.
    • Must be able to read, write, and understand the English language and speak the language in an understandable manner.
    • Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be met fully.
    • Experience with working with Windows based software.
