    The primary purpose of this position is to assist residents of Wilmington and Blanchester Affordable Living communities (Prairie View, Quaker Place, Friendly Center and Mulberry Place as well as Blanchester Friends) in accessing telehealth behavioral health services.  Involves direct care with people experiencing mental health disorders.  Requires ability to interact with empathy and sensitivity. This is a pilot project with Xavier University’s School of Nursing and is a time-limited, grant-funded position that will end in June 2019.

     

    Affordable Living at ERS: Our philosophy of care guides us to maintain every person’s dignity, and provide the same standard of excellence in care to every resident. Our Affordable Living communities are wonderful places for older adults to live and flourish in safe, friendly neighborhood communities. Residents enjoy amenities and services found in few retirement communities for people with low-incomes such as transportation, gardening areas, complimentary laundry facilities, and chaplain services. The older adults who live in our Affordable Living communities are given the opportunity to live and flourish in a friendly, loving environment.

     

    Responsibilities

    • Promotes interest in telehealth pilot program through in-person meetings and flyers.
    • Collaborates with Service Coordinator to recruit appropriate residents for program participation.
    • Accurately schedules appointments for residents and provides needed support, such as reminder and follow-up calls, to residents for program adherence. 
    • Works collaboratively and effectively with a team.  Builds and maintains positive, supportive relationships with residents and other team members. Communicates with students, faculty, residents, Service Coordinators, and other team members as needed for program success.
    • Ensures compliance with program specifications, such as maintaining consent forms and other records of information needed for documentation.  Responsible for collecting, tracking, and reporting metrics and data related to the pilot project.
    • Other responsibilities as deemed necessary and appropriate, or as directed.
    • Attends and participates in workshops, in-services, seminars, etc.

    Qualifications

    • High School Diploma or GED, Associate’s Degree preferred.
    • Previous experience developing relationships with residents and community agencies.
    • Must enjoy and have excellent skills in working with people.
    • Must be able to work independently with little supervision and able to handle multiple tasks while meeting deadlines.
    • Must possess the ability to make independent decisions and to accept constructive criticism.
